Job summary

Sigma Health Rehab is actively seeking a full-time Occupational Therapist (OT) Treating Supervisor to work in a Skilled Nursing Facility. We offer a $5,000 Sign-On Bonus for this position.

The OT will evaluate residents, develop individualized treatment plans, and support daily activities to enhance independence while collaborating with the healthcare team. NY license and NY OT program graduation are required; SNF/geriatric experience is preferred.

Qualifications

• Must be a graduate of an accredited OT program.
• Current New York State OT License
• Experience in skilled nursing or geriatric care preferred

Responsibilities

• Evaluate residents and develop OT treatment plans meeting their needs.
• Choose activities to help residents perform daily tasks independently.
• Observe, assess, and document progress with detailed reports.
• Maintain resident confidentiality in information processing.
• Foster cooperation within the healthcare team
