Job summary

PruittHealth is seeking a licensed Occupational Therapist (OT) for our Sylvester SNF location. This full-time, part-time, or PRN opportunity offers the chance to help patients regain independence and improve daily living skills in a post-acute care environment.

The role includes evaluating ADLs, developing treatment plans, recommending adaptive equipment, and collaborating with care teams. A state license and accredited OT program completion are required, with experience in SNF or post-acute

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state.
• Experience in rehabilitation, SNF, home health, or post-acute care preferred.
• Strong problem-solving and communication skills.
• Foreign-trained applicants must possess credentials deemed substantially equivalent by the credentialing organization.

Responsibilities

• Evaluate patients' ability to perform activities of daily living (ADLs).
• Develop treatment plans that enhance independence and functional performance.
• Recommend adaptive equipment and environmental modifications.
• Address cognitive, physical, and sensory limitations impacting daily function.
• Document patient progress and treatment outcomes.
• Collaborate with interdisciplinary healthcare teams.
