Job summary

Evolve Therapy Services, LLC in Church Hill, TN seeks an Occupational Therapist for an in-house Skilled Nursing Facility. This PRN role emphasizes planning, directing Occupational Therapy services, and maintaining high-quality resident care.

You will screen residents, evaluate and develop treatment plans, communicate findings to families and the interdisciplinary team, and coordinate therapy schedules while ensuring proper documentation and billing.

Qualifications

• Must hold a Degree in Occupational Therapy and an active OT license.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop short- and long-term goals with treatment plans.
• Interpret and communicate findings to residents, families, and the care team.
• Establish individual therapy schedules with the team.
• Document therapy services and ensure proper billing.
• Oversee COTA and ensure compliant treatments and documentation.
