Job summary

Rivetus Rehabilitation is seeking an Occupational Therapist to join a hybrid SNF/outpatient practice in Troy, MI. You will provide one‑on‑one, concurrent, and group therapy across settings and discharge patients with tailored plans.

You will work with a team of licensed clinicians to ensure continuity of care and support program development while maintaining documentation and compliance with policies.

Qualifications

• Successful completion of an accredited occupational therapy program.
• Licensure as required by the State of Michigan.

Responsibilities

• Conduct occupational therapy screenings and evaluations to determine patient independence with activities of daily living.
• Provide occupational therapy treatments consistent with AOTA standards.
• Develop and implement treatment plans using adaptations, strengthening, and retraining to maximize patient independence.
• Deliver direct patient care in accordance with productivity standards, using graded activities to restore physical and functional abilities.
• Check, instruct, assign, and review the work of OTAs and aides as assigned.
• Maintain accurate medical records that support quality, continuity of care, and reimbursement guidelines.
• Participate in patient care conferences and company meetings as directed.
• Engage in continuous quality improvement activities.
• Promote current clinical practice standards through continuing education.

Job description

As an OT in this role, you will become an integral teammate in our continuum of care, where we offer skilled nursing facility (SNF) therapy as well as one‑on‑one outpatient therapy to patients from outside of the SNF.

Why choose the hybrid role of SNF/OP?

This position is great for any therapist—from a new graduate who wants to gain exposure to a variety of settings to a seasoned clinician excited about providing a wide range of treatments. The SNF/OP role allows you to flex your treatment muscles and see a wide range of diagnoses and treatment approaches. In the SNF, you can expect to provide one‑on‑one treatments, concurrent therapy, and group therapy as appropriate; on the outpatient side you will deliver individualized 45‑60 minute sessions to patients who recently discharged from the facility or who come from the community. The schedule adjusts with SNF caseloads, so you won’t have to worry about inconsistent hours.

Rivetus has been serving the geriatric post‑acute community since 1998. Our company is operated and managed at the executive level by licensed clinicians, providing continuity of care across skilled nursing, home health, and outpatient settings.

General Summary

In this role you will provide occupational therapy services to patients with physical or cognitive dysfunction, including assessment, treatment planning, and implementation. You will participate in patient care conferences, program development, and screenings as appropriate; provide supervision for OTAs and aides; complete documentation in line with policies, procedures, and current standards of clinical practice.

Your Responsibilities

Required Qualifications
