Position Overview

Full Time Occupational Therapist (OT) – Home Health

Anticipated Territory: Santa Clara County with multiple small coverage areas.

What You’ll Do

In partnership with the Case Manager, you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively. Responsibilities include home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease and dementia management, and family caregiver training. You will serve as an educator, patient advocate, and member of the interdisciplinary care team.

Qualifications & Experience

• Current unrestricted California Occupational Therapist license
• Current BLS/CPR certification
• Valid California driver’s license
• Knowledge of local community resources
• Understanding of individual development, human behaviour, and cultural influences on health care
• Knowledge of state and federal home‑care regulatory guidelines preferred
• Experience in home health care highly preferred
• Experience with high‑acuity patients is a plus
• Experience working with interdisciplinary teams desired
• Case management fundamentals desired
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized

Pay & Benefits

Base pay range: $118,000 – $130,000 per year, commensurate with skills and experience. Starting pay above minimum requirements. Generous bonus incentive plans, comprehensive health benefits, and 401k up to 4% match.
