Job summary

FOX Rehabilitation in Riverdale, NY, is seeking a licensed Occupational Therapist to deliver one-on-one care in homes or senior living communities. You will assess functional mobility, develop individualized treatment plans, and help older adults improve independence and quality of life through the Geriatric House Calls model.

In addition to direct patient care, you may use telehealth to extend access and flexibility, collaborating with an interdisciplinary team and managing your own schedule.

Qualifications

• Active OT license in New York or eligibility to obtain.
• Degree from accredited OT program.
• Proficient with EMR systems and telehealth tools.
• CPR certification.

Responsibilities

• Provide one-on-one Occupational Therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).

Responsibilities

• Provide one‑on‑one Occupational Therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Occupational Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

• Exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Perform CPR
• Provide moderate to maximum assistance with patient transfers and bed mobility
• Reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wear PPE when required

In New York, the standard base pay range for a Full‑Time role is $70,000 – $105,000 annually with an opportunity to earn more. Part‑time and FOX Flex (PRN) will be pro‑rated.
