Job summary

Sweetwater Care in Phoenix, AZ seeks a licensed Occupational Therapist to develop personalized treatment plans, perform assessments, and guide patients through therapeutic exercises to improve mobility and activities of daily living.

You will document patient progress in the electronic records, collaborate with the rehabilitation team, supervise assistants, and communicate with patients and families about discharge planning and expectations.

Qualifications

• Active Arizona OT license required.
• Master's degree in Occupational Therapy or equivalent.
• Experience with evidence-based practice and patient education.

Responsibilities

• Perform treatments and exercises to aid recovery.
• Develop individualized treatment plans.
• Document goals and progress in EHR per regulatory requirements.
• Supervise and support assistants and students where applicable.
• Communicate treatment plans and progress with patients, families, and health team.
• Collaborate with rehabilitation team and report to Director of Rehabilitation Services.
