Job summary

FutureCare Health & Management in Maryland is seeking an Occupational Therapist to join our interdisciplinary rehabilitation team. You will assess residents, develop and implement individualized treatment plans, and guide residents and families to maximize functional abilities.

Under the Rehab Director, you will document progress, comply with regulations, supervise assistants, and participate in quality improvement activities. MD license and at least 1 year of rehab experience required.

Qualifications

• Bachelor of Science Degree in Occupational Therapy
• Current Maryland OT license/certification
• Minimum of one year experience in rehabilitation
• Supervision/training experience in long-term care or sub-acute setting preferred
• Demonstrated knowledge of accreditation standards and compliance requirements

Responsibilities

• Review resident information and evaluate impact on assessment and treatment
• Conduct and document thorough functional assessment using standardized procedures
• Provide assessment of home/vocational and equipment needs
• Develop and implement an individualized treatment plan with achievable goals
• Comply with evaluation, treatment and documentation guidelines
• Document the course of treatment in the resident's medical record
• Revise treatment plans as needed and supervise assistants
• Orient residents to program services and inform about risks
• Collaborate with nursing staff and families to maximize resident function
• Assist in referrals for additional services after discharge
• Maintain knowledge of community resources
• Attend department/facility meetings and participate in quality improvement and safety programs

Education

Bachelor of Science Degree in Occupational Therapy

Current Maryland Occupational Therapist license/certification
