Job summary

Glengariff Rehabilitation and Healthcare Center in Jericho, NY seeks a skilled Occupational Therapist (OT) to join our rehabilitation team. The OT will assess referred patients, develop and administer treatment plans, and document progress under physician oversight.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Perform evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Excellent patient service skills and clinical excellence.

Responsibilities

• Assess referred patients requiring rehabilitation services and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document and educate patients and families on therapy methods.
• Work under physician direction to ensure safe and effective care.
