Advocate Health Care in Elgin, Illinois is looking for an Occupational Therapist to evaluate and treat patients to enhance their functional independence. This position involves developing individualized treatment plans and collaborating with healthcare teams to ensure optimal patient care.

Interprets physician referrals and evaluates patients’ physical, perceptual, cognitive, social, and vocational abilities and their impact on activities of daily living; establishes a plan of care and implements individualized treatment; provides and/or fabricates assistive devices and splints to enhance functional independence; monitors and evaluates outcomes and adjusts treatment plans as needed; attends to factors affecting physical and psychosocial comfort; plans and completes patient discharge with home programs and referral to community services; collaborates with stakeholders to optimise care; completes required documentation in accordance with department policy and accreditation standards; maintains productivity and billing standards; cleans up after treatment and maintains a neat work area; performs manual techniques, patient positioning, transferring, and transportation using proper technique; demonstrates skill in modalities, exercise equipment, and other devices; promotes patient safety and follows infection control measures; participates in performance improvement activities; manages caseload and informs leadership of coverage needs; pursues professional growth through continuing education; orients new staff; participates in evaluation of co‑workers; and may supervise occupational therapy assistants, extenders, or students as appropriate.
