Snap Health in Arlington, TX is seeking a licensed Occupational Therapist for a contract, PRN/home health role. You will evaluate patients, develop and implement plans of care, and coordinate with the health care team to ensure optimal outcomes.

The role requires Texas OT licensure, CPR certification, a valid driver's license, and comfort with mobile apps. Flexible caseload and opportunities for extra hours are available.

Qualifications

• Must hold valid Texas OT license.
• CPR certification required.
• Valid driver’s license, auto insurance and professional liability insurance.
• Experience in home health is a plus.
• Able to work independently and as part of a team.
• Proficient in using mobile apps for organization.

Responsabilités

• Evaluates patient functional status and OT needs; assists physician in plan of care.
• Provides services within OT scope of practice and plan of care.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patients and caregivers about plan of care and progress.
• Visits patients according to Plan of Care and completes progress notes.
• Supervises COTAs.
• Participates in discharge planning.

This is a contract role for an Occupational Therapist at Snap Health in Arlington, TX & Surrounding Areas. As part of the team, you will be providing patient care in a Home Health, on-site setting. Your day-to-day tasks will involve delivering quality care and ensuring optimal patient outcomes.

Responsibilities

• Evaluates the patient’s functional status and occupational therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by the state board of occupational therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Treats patients to relieve pain, restore function, and / or maintain maximum functioning using acceptable standards of practice.
• Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Care, completes progress notes timely.
• Provides supervision of COTAs
• Participates in discharge planning for patient.

Qualifications

• Must be currently licensed to practice as an Occupational Therapist in the STATE OF TEXAS
• Must possess and maintain valid CPR certification.
• Valid Driver’s License, Auto Insurance, Professional Liability and Reliable Transportation
• Experience in a home healthcare setting is a plus
• Ability to work independently and as part of a team.
• Proficient in using mobile apps for organizational purposes.

Job Types: PRN, Contract

Pay:$65.00 – $85.00 per hour
