Job summary

Jefferson Health Plans is seeking an Occupational Therapist to evaluate, treat, and discharge plan for patients across acute care, skilled facilities, rehab, and outpatient settings.

You will collaborate with a multidisciplinary team, document progress, and ensure compliance with NBCOT and state licensure requirements. The role supports Jefferson Health's mission and requires BLS/CPR certification.

Qualifications

• Bachelor’s, Master’s or Doctoral Degree from an accredited Occupational Therapy Program.
• Valid state license in Occupational Therapy; NBCOT certification required.
• BLS/CPR certification required via AHA.

Responsibilities

• Evaluate, treat and discharge plan for a full patient caseload across acute care, skilled facility, acute rehab and outpatient settings.
• Plan, organize and deliver treatment programs to aid recovery and rehabilitation.
• Utilize tools and techniques for safe therapeutic interventions and trainings for patients and families.
• Record and document health progress, daily treatments, evaluations and discharge summaries.
• Perform other duties as assigned.

Job Details

The Occupational Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their given services. The therapist is an integral healthcare provider accountable to assist the patient to their highest level of function.

The therapist will uphold the mission, vision, and values of Jefferson Health.

Works in conjunction with other health care professionals to coordinate treatment plans and care.

Essential Functions:

· Provide therapy evaluations, treatment planning, treatment interventions and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, outpatient settings as appropriate.

· Plans, organizes and delivers treatment programs that help clients/patients recover and rehabilitate.

· Utilizes tools and techniques to administer safe therapeutic interventions and trainings, for patient and families.

· Records and documents clients/patients’ health progress information for evaluation, all therapeutic interventions including daily treatments, progress notes, evaluations and discharge summaries.

· Performs other duties as assigned

Job Description

Bachelor’s, Master’s or Doctoral Degree from an accredited Occupational Therapy Program

Valid state license in Occupational Therapy. Valid certification from the National Board of Certification in Occupational Therapy (NBCOT) is required.

BLS, CPR certification required via AHA.
