Job summary

Vertis Therapy is seeking a Part Time or PRN Occupational Therapist to work with elders in assisted living facilities in the South Bend and Mishawaka, IN area. The OT will develop individualized treatment plans, provide skilled services, and supervise COTAs as needed, with emphasis on lymphedema experience preferred but not required.

Responsibilities include patient assessment, documentation, supervision, professional development, and leading wellness programs to improve residents’ independence

Qualifications

• Education: Degree in Occupational Therapy from an accredited institution.
• Licensure: Valid Occupational Therapist state license or license eligibility.
• Skills: Strong clinical assessment, proficient in developing and implementing effective treatment plans, excellent communication and interpersonal skills.
• Physical: Ability to lift and stand for extended periods while performing therapy.
• Technology: Computer proficiency to document in electronic medical record platform.

• Complete timely and accurate documentation.

Vertis Therapy is looking for a passionate Part Time or Consistent PRN Occupational Therapist who loves working with elders for our Assisted Livings with memory care in the South Bend and Mishawaka, IN area. Vertis Therapy is a therapist owned company that focuses on providing quality, patient centered care. We are especially interested in candidates with lymphedema certification or specialized experience in lymphedema management, although this is not required for the position.

The Occupational Therapist will develop individualized treatment plans, provide skilled occupational therapy services, monitor patient progress, and collaborate with the interdisciplinary team. The OT will also provide guidance and supervision to Certified Occupational Therapy Assistants (COTAs) as appropriate.

Responsibilities

• Patient Assessment and Treatment: Evaluate residents' functional abilities. Establish and execute a plan of care based on individual goals to improve resident's independence and quality of life.
• Documentation: Complete timely and accurate documentation.
• Supervision: Provide appropriate guidance and supervision to Certified Occupational Therapy Assistants.
• Professional Development: Participate or lead team development meetings.
• Wellness Programming: Regularly lead community wide wellness programs and events.

Qualifications

• Education: Degree in Occupational Therapy from an accredited institution. Prefer current registration with the American Occupational Therapy Certification Board.
• Licensure: Valid Occupational Therapist state license or license eligibility
• Skills: Strong clinical assessment skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills. Ability to work independently and in a team environment.
• Physical Requirements: Ability to lift and assist clients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
• Technology: Computer proficiency to document in electronic medical record platform.
