Job summary

Omega Home Health Care Services Inc is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis in California. You will provide rehabilitative services to patients in their homes, develop treatment plans, and educate patients and caregivers on adaptive equipment and daily living skills.

Requirements include a California OT license, at least 1 year of home health experience preferred, reliable transportation, and excellent communication.

Qualifications

• Current and valid Occupational Therapist license in California.
• Graduate of an accredited Occupational Therapy program.
• Minimum of 1 year experience in a home health or similar setting preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with EMR systems is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver occupational therapy services in patients’ homes in accordance with physician orders and company protocols
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills
• Monitor patient progress and modify treatment plans as needed
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers
• Maintain accurate and timely documentation in compliance with California state and federal regulations
• Ensure patient safety and adherence to infection control procedures in home environments

• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills

This position is paid by the visit. Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
