Job summary

Sans Souci Rehabilitation and Nursing Center is seeking an Occupational Therapist to join our team. The OT will assess patients, develop and administer individualized treatment plans, and set realistic goals based on each patient’s capacity and tolerance.

Supervision of OT Assistants and strong documentation are essential. Responsibilities include evaluations, progress notes, discharge planning, and collaborating with all disciplines to deliver patient-centered care in a long-term care setting.

Qualifications

  • Graduated from an accredited OT program and licensed to practice.
  • Valid OT license in the state of New York.
  • Experience supervising OT Assistants.
  • Experience in long-term care/geriatric settings preferred.
  • Strong documentation and record-keeping skills.
  • Experience with evaluations, progress notes, re-certifications and discharge documents.

Responsibilities

  • Assess referred patients requiring rehabilitation and provide direct patient care.
  • Establish and administer a treatment program with goals based on patient capacity.
  • Set realistic and achievable goals and document patient status.
  • Educate patients and families on therapy methods.
  • Collaborate with other disciplines to provide patient-centered care.
  • Maintain confidentiality and HIPAA compliance.

Occupational Therapist

The Sans Souci Rehabilitation & Nursing Center is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:
  • Must have graduated from an accredited Occupational Therapist college or university.
  • Must have a valid professional license and registration for state of practice.
  • The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, re-certification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekend

Salary up to: $55.00/hour
