Job summary

Valley Manor is seeking a compassionate Occupational Therapist (OTR) to evaluate, plan, and implement individualized treatment programs aimed at maximizing independence and quality of life for patients. You will assess functional abilities and develop plans while collaborating with a multidisciplinary team in a skilled care setting.

Qualified candidates hold an accredited OT program degree and a current state OTR license, with strong communication, time management, and organizational skills.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current state Occupational Therapist license (OTR).
• Strong communication, time management, and organization skills.
• Ability to work independently and as part of a team.
• Lymphedema Management certification/experience.

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans
• Provide skilled occupational therapy services in accordance with physician orders and plan of care
• Address ADLs, IADLs, mobility, strength, coordination, cognition, and adaptive equipment needs
• Educate patients, families, and caregivers on treatment goals, progress, and home programs
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Collaborate with physicians, nursing staff, and interdisciplinary team members
• Maintain compliance with state regulations, facility procedures, and ethical standards
