Job summary

The Creekside Center for Rehabilitation and Healing is seeking an Occupational Therapist to join our team. You will assess referred patients, develop and implement rehabilitation programs, and set achievable goals in collaboration with physicians.

You will document patient progress, educate patients and families, supervise OT Assistants, and work with a multidisciplinary team to deliver high-quality, patient-centered care in a long-term care setting.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Licensed and registered to practice OT in the state.
• Oversee and supervise OT Assistants.
• Experience with long-term care or geriatric settings preferred.
• Strong documentation and record-keeping skills.
• Performs evaluations, progress notes, re-certifications, and discharge documentation.
• Collaborates with all disciplines for patient-centered care.
• Demonstrates professionalism and punctuality.
• Adheres to confidentiality, HIPAA, and Residents' Rights.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Develop and implement individualized treatment plans with goals.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
