Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking an Occupational Therapist to join an interdisciplinary care team. You will evaluate residents, plan and implement therapy to improve ADLs, safety and independence, and collaborate with PT, SLP, nursing, and dietary staff to support discharge readiness.

New graduates are welcome; structured mentorship, on-site clinical education, and advancement opportunities across our 35-facility network are available.

Qualifications

• Licensed occupational therapist with eligibility to practice in NY.
• Ability to supervise occupational therapy assistants per state rules.
• Experience in long-term care or geriatric rehab is a plus.

Responsibilities

• Evaluate referred residents to determine medical status and rehab potential.
• Build individualized treatment plans with measurable goals.
• Deliver OT interventions focused on ADL performance and safety.
• Complete evaluations, progress notes, recertifications, and discharge summaries.
• Educate residents and families on therapy techniques and carryover strategies.
• Collaborate with PT, SLP, nursing, and dietitians for discharge readiness.
• Recommend adaptive equipment and environmental modifications.
• Supervise OTAs and students in accordance with state practice act.

The Chateau at Brooklyn Rehabilitation & Nursing Center in Brooklyn, NY is 189-bed rehabilitation center offering comprehensive short- and long-term rehabilitation programs, including PT, OT, ST, and respiratory services, a spacious rehabilitation gym, free on-site parking, and convenient access to public transportation, with the B36 bus stopping right in front and the B43 and B3 buses just one block away.. Hiring for this role is done on site by our Director of Rehab and facility leadership, not by an outside recruiter. You will interview with the people you will actually work with.

What You Will Do

• Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential

• Build individualized treatment plans with measurable, achievable goals based on each resident’s capacity and tolerance, under physician direction

• Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function

• Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time

• Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies

• Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient centered care and support discharge readiness

• Recommend adaptive equipment, durable medical equipment, and environmental modifications

• Supervise Occupational Therapy Assistants and students in accordance with the state practice act

What You Will Need

• Degree in Occupational Therapy from an accredited program

• Valid Occupational Therapist license and registration in the state of practice, or license eligible

• Ability to supervise Occupational Therapy Assistants in accordance with the state practice act

• Comfort with clinical documentation including evaluations, weekly progress notes, recertifications, and discharge summaries

• Commitment to confidentiality, HIPAA, and Federal Resident Rights

Preferred, Not Required

• Experience in long term care, sub acute, or geriatric rehabilitation

• Prior experience in an interdisciplinary care model

• New graduates and therapists new to skilled nursing are encouraged to apply. We train.
