Job summary

Devotion Hospice is seeking an Occupational Therapist (PRN) to provide occupational therapy services across patient age groups. The OT will assess, plan, implement and evaluate care plans to restore or maintain patient well-being, and participate in the IDG care planning process.

The role requires a valid OT license, CPR certification, and a valid driver’s license with auto insurance. Previous HomeCare HomeBase experience is preferred. Physical demands may apply as part of direct patient care.

Qualifications

• Valid Occupational Therapy License.
• 1 year OT experience preferred.
• CPR certification required.
• Valid Driver’s License & Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform clinical assessment; identify patient needs and appropriateness of care.
• Document, prepare, revise and complete Plan of Care, progress notes, phone orders and other clinical records in a timely manner.
• Accept clinical assignments consistent with education and competence to care for patients/clients.
• Participate in the IDG care planning process.
• Apply infection control concepts and standard precautions in patient care activities.
• Assess and facilitate ROM, strength, coordination, adaptive equipment needs, mobility and safety related to ADLs; fabricate/maintain adaptive equipment and splints.
• Use interpersonal skills to coordinate with the team for desirable outcomes.
• Evaluate patient occupational performance in self-care, work, leisure and rest; assess sensory-motor, psycho-social and cognitive-perceptual components.

The Occupational Therapist is a licensed professional who provides occupational therapy to patients/clients of all age groups. Plans, implements and evaluates patient/client care plans to restore or maintain patient/client well being. Evaluates occupational roles and occupational performance of patients/clients with physical, psycho-social and/or cognitive impairments. Provides interventions for deficits and elicits responses. Participates in the coordination of care.

As a Occupational Therapist , you will:

Perform clinical assessment; identifies patient needs and appropriateness of patient.

Documents, prepares, revises, and completes a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.

Accepts clinical assignments that are consistent with education and competence to care for patients/clients.

Participates in the IDG care planning process.

Apply concepts of infection control and standard precautions in coordination/performing patient care activities to prevent contamination and transmission of disease.

Assess and facilitate through direct therapeutic intervention the patient's ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patient’s ADL performance needs. Fabricate and maintain adaptive equipment and hand splints.

Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.

Evaluate patient occupational performance in the areas of self-care, work, leisure and rest. Assess sensory-motor neuromuscular, psycho-social and cognitive-perceptual components of occupational behavior. Assess physical, social and cultural components of the patient’s occupational environment.

Meets mandatory continuing education requirements of the Agency and the licensing board.

Promotes the Agency philosophy and administrative policies to ensure quality of care.

Requirements

Valid Occupational Therapy License

1 year of occupational therapy experience preferred

CPR certification
