Job summary

The Encore at Boca Raton Rehabilitation and Nursing Center is seeking an Occupational Therapist to join our team. You will assess referred patients, develop treatment plans, and deliver direct rehabilitation services in a long-term care setting.

Ideal candidates will have an accredited OT degree, a valid state license, and experience supervising OT Assistants. Strong documentation skills and a patient-centered approach are essential for success in this role.

Qualifications

• Graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation writing and ability to produce evaluations and discharge documents.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Maintains punctuality and professionalism; adheres to confidentiality and residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment goals determined by patient capacity and physician direction.
• Document patient condition and educate patients and families in therapy methods.
• Ensure timely reporting, weekly progress notes, and discharge documentation.
• Maintain confidentiality and comply with HIPAA and Residents' Rights.

Occupational Therapist

The Encore at Boca Raton Rehabilitation and Nursing Center is looking for a talented and hard-working Occupational Therapist to join our team. Today, under new ownership, we are creating an inimitable, revitalizing Rehabilitation Environment to provide the short term patient with a luxurious, five-star experience that meets and exceeds all expectations.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
