A healthcare organization is looking for a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting. You will help patients regain independence and improve daily functioning through individualized treatment plans while collaborating with interdisciplinary teams. Ideal candidates should hold a Bachelor's degree in Occupational Therapy and have strong communication and organizational skills. Experience in LTC and knowledge of Medicare documentation is preferred.

Formación

• Licensed Occupational Therapist with current state OT license.
• LTC experience preferred.

Occupational Therapist (OT) – Long-Term Care

LP Therapy

LP Therapy is seeking a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting, helping patients regain independence and improve daily functioning.

Responsibilities: review medical records; evaluate patient abilities and needs; develop and implement individualized treatment plans; provide ADL training, adaptive equipment, and environmental modifications; document progress per Medicare guidelines; collaborate with interdisciplinary teams; supervise COTAs as assigned; support patient satisfaction and quality outcomes.

Qualifications: Bachelor's degree in Occupational Therapy; current state OT license; LTC experience preferred; strong knowledge of Medicare documentation; excellent communication and organizational skills.

Work Environment: LTC facility; physical demands consistent with therapy services.
