Job summary

Enhabit Home Health & Hospice is seeking a licensed Occupational Therapist to join our home-based care team in Arizona. You will assess patients, develop daily living skills, and implement therapy plans to improve function at home.

The role emphasizes compassionate care, adherence to Medicare guidelines, and collaboration with regional leadership. Prior home health experience is preferred; licensure is required.

Qualifications

• Must be a graduate of an approved school of Occupational Therapy and licensed as an OT in the practicing state.
• A minimum of one-year experience is preferred, but can be less with regional leadership approval based on prior experience in setting.
• Medicare home health experience is preferred.

Responsibilities

• Help patients in the home improve daily living skills and basic motor functions.
• Use treatments to develop daily living skills and compensate for loss of function.
• Collaborate with care teams to implement therapy plans and document progress.

Responsibilities

Our Occupational Therapists help patients who have debilitating conditions improve the functions of performing everyday tasks in the home. Occupational therapists use treatments to develop the daily living skills of their patients, the basic motor functions of patients as well as to compensate for any loss of function that may have occurred with the patient.

Qualifications

• Must be a graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist/OT in the state in which he/she currently practices.
• A minimum of one-year experience is preferred, but can be less with Regional Vice President and/or Regional Director of Therapy Services approval based on prior experience setting.
• Medicare home health experience is preferred.
