Job summary

ABL Health Care, LLC in the San Mateo area seeks a Full Time Occupational Therapist (OT) to join our Home Health team. You will assess patients, develop and implement care plans, and coordinate with our Clinical Director and staff to deliver high-quality care in patients' homes across San Mateo, Burlingame, and South San Francisco.

This role requires a California OT license, at least 1 year of home health or hospice experience, a valid CA driver’s license, and willingness to travel 15–25 miles.

• Willingness to travel 15–25 miles regularly.

Responsibilities

• Provide skilled intermittent care in patients' homes and educate family/caregivers.
• Evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to the care team.
• Develop, implement, and revise the plan of care (POC).
• Maintain accurate clinical records and document per policy.
• Coordinate with other disciplines and communicate as needed.

Position Overview

We are seeking an experienced, self-driven Full TimeOccupational Therapist (OT)to join our dedicated Home Health team and provide exceptional care throughout the region. As an Physical Therapist (PT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current OT License to practice in California -Required
• Experience in Home Health or Hospice care -1 Year Required
• Current CA Driver's License
• Able to travel within a 15 - 25 mile radius on a regular basis.
