Klarus Home Health is seeking a licensed Occupational Therapist to provide full-time, part-time, or PRN services in the South Dallas area. You will conduct OASIS assessments, develop care plans, and deliver one-on-one home health OT to patients in Dallas, Duncanville, Cedar Hill, Lancaster, Desoto, Grand Prairie, and nearby communities.

Qualifikationen

• Bachelor of Science in Occupational Therapy required.
• OT license active in state; valid driver’s license required.
• Minimum 1 year OT experience; home health experience preferred.
• EMR proficiency in Home Care/HH base preferred.
• CPR certification and knowledge of durable medical equipment.
• Reliable transportation and valid auto liability insurance.

Aufgaben

• Conduct ongoing clinical assessments, including OASIS, to identify patient needs and determine functioning levels.
• Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
• Assess sensory-motor, psycho-social, and cognitive-perceptual components of occupational behavior.
• Evaluate environmental factors of patients’ occupational environment.
• ProvideDirect therapeutic interventions addressing ROM, strength, coordination, adaptive equipment, and home modifications.
• Develop, evaluate, and revise the plan of care with patients, representatives, and caregivers.
• Document clinical notes, phone orders, and other records promptly.
• Accept assignments consistent with education and competence.
• Provide physician-ordered counseling and education for patients, caregivers, and families.
• Supervise and guide OT assistants and home health aides as appropriate.
• Apply infection control and standard precautions to prevent contamination.
• Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.

Requirements

Key Responsibilities:

• Conduct ongoing clinical assessments, including OASIS assessments, to identify patient/client needs and determine levels of functioning.

• Evaluate physical, social, and cultural components of patients’ occupational environment.
• Facilitate direct therapeutic interventions to address ROM, strength, coordination, adaptive equipment needs, and environmental modifications.
• Develop, evaluate, and revise the plan of care in collaboration with patients, representatives, and caregivers.
• Document clinical notes, phone orders, and other records in a timely manner.
• Accept clinical assignments consistent with education and competence.
• Provide physician-ordered services, including patient/client, caregiver, and family counseling and education.
• Supervise and guide occupational therapy assistants and home health aides as appropriate.
• Apply infection control and standard precautions to prevent contamination and transmission of disease.
• Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.

Job Qualifications:

• Education: Bachelor of Science in Occupational Therapy.
• Licensure: Current State License as an Occupational Therapist, Current driver’s license required.
• Experience: One year of experience as an Occupational Therapist preferred. Home health experience preferred
• EMR: Home care home base proficiency preferred
• Skills: Proficiency in therapy skills as per accepted standards of practice, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment.
• Transportation: Reliable transportation and valid/current auto liability insurance.
