Job summary

Liberty Healthcare and Rehabilitation Services is seeking an Occupational Therapist (OT) at Liberty Cares Falcon, NC. You will work with a multidisciplinary team to tailor rehab plans for patients and families, delivering individual, progressive OT interventions in-house with local management support.

The role requires completion of an accredited therapy program, state licensure for the SNF location, and preferred prior rehab SNF experience to ensure high-quality patient outcomes.

Qualifications

• Graduate of an accredited therapy program.
• Must be licensed in the state where the SNF is located.
• Previous rehab SNF experience preferred.

Responsibilities

OCCUPATIONAL THERAPIST (OT)

Work with a multidisciplinary team to insure a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans. Be able to work in a supportive rehab environment with progressive treatment modalities to address a variety of diseases and impairments. In-house therapy with local management support.

Job Requirements:

EDUCATION: Must be a graduate of an accredited Therapy program.

CERTIFICATE / LICENSE: Must be licensed in the state of the SNF you will be working.

EXPERIENCE: Previous rehab SNF experience is preferred.
