Job summary

The Savoy at Fort Lauderdale Rehabilitation and Nursing Center is seeking an Occupational Therapist to assess referred patients, develop and administer rehabilitation plans, and document outcomes under physician direction.

The role emphasizes setting achievable goals, educating patients and families, and collaborating with a multidisciplinary team to deliver high-quality, patient-centered care in a long-term care setting.

Qualifications

• Graduated from an accredited OT program and holds current OT license/registration.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and charting skills (evaluations, progress notes, re-certifications, discharges).
• Ability to collaborate with multidisciplinary teams and provide patient-centered care.
• Must maintain confidentiality and HIPAA compliance.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential for rehabilitation.
• Develop and administer individualized treatment plans with goals set to patient capacity under physician direction.
• Set realistic, achievable goals and document patient conditions and progress.
• Educate patients and families on therapy approaches and home programs.
• Supervise and provide guidance to Occupational Therapy Assistants.
• Maintain timely documentation and discharge planning as required.

Occupational Therapist

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
