Job summary

Sans Souci Rehabilitation and Nursing Center is seeking an Occupational Therapist to assess and treat patients in a long-term care setting. You will develop care plans, document outcomes, and educate patients and families in therapy methods.

The role requires an accredited OT program completion, valid state license, and experience in geriatric care. You will supervise OT Assistants and collaborate across disciplines to deliver patient-centered care.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must hold a valid OT license and state registration.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term care or geriatric care preferred.
• Strong documentation writing skills and timely charting.
• Complete evaluations, progress notes, re-certifications and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Maintain confidentiality, HIPAA compliance, and residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation and deliver direct patient care.
• Develop and administer treatment plans with goals aligned to patient capacity.
• Document patient conditions and educate patients and families on therapy methods.
• Supervise and guide OT Assistants and ensure quality care.

Occupational Therapist

The Sans Souci Rehabilitation & Nursing Centeris looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: All positions are available.

Salary $45 an hour
