Renewal Rehab is seeking a licensed Occupational Therapist for a full-time position in Streator, IL at a skilled nursing facility. The role focuses on evaluating and delivering OT services to improve patients’ functional independence and quality of life.

You will collaborate with a multidisciplinary team, maintain compliant documentation, and advance professional growth through ongoing education and programs offered by the Enhance Therapies family.

Qualifications

• OT degree from an AOTA-accredited program.
• NBCOT certification as an OT (OTR) if required by the state.
• Current license as an Occupational Therapist in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
