Job summary

Rehab Without Walls Neuro Rehabilitation in San Jose, CA is seeking an experienced Occupational Therapist for a flexible PRN, W2 role. You will serve as the primary therapist from evaluation through discharge, delivering real-world, home- and community-based rehabilitation.

This position offers autonomy with collaboration from an interdisciplinary team of OT, PT, SLP, and social work colleagues, about 15–20 hours weekly across three days, with hourly pay and no weekend shifts.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited program.
• Current, unrestricted license as an Occupational Therapist in the practicing state.
• Current CPR Certification.
• A minimum of one year’s work experience as an Occupational Therapist.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans and participate in staffing meetings and family conferences.
• Perform discharge evaluations and make recommendations for home and community safety; provide resources.
• Communicate patient needs and progress to the treatment team, physician, patient, and family.
• Collaborate with other disciplines to ensure coordinated care and better outcomes.

• An experienced Occupational Therapist interested in serving as a primary therapist from evaluation through discharge.
• Availability for approximately 15–20 hours per week across 3 days, with consistent client relationships.

  • Managing and picking up their own prescriptions

Responsibilities

What you will do: Responsibilities listed include but not limited to:

• Perform evaluations and develop treatment plans
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
• Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes

Qualifications

What you will need:

• Minimum of a bachelor’s degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program
• Current, unrestricted license as an Occupational Therapist by state in which practicing
• Current CPR Certification
• A minimum of one year’s work experience as an Occupational Therapist
• Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses preferred
• Communicates effectively and professionally in verbal and written interactions
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills,visual acuity,and walking/standing for extended periods
• Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
• A health screen or examination may be required prior to assignment and periodically thereafter, depending on specific location policy, local and state regulations to verify employee is physically capable of performing assigned duties with or without reasonable accommodations
