Job summary

Luxor at Mills Pond Rehabilitation & Nursing Center seeks a skilled Occupational Therapist to join our evolving care team in a long-term care setting. You will assess referred patients, develop and direct individualized therapy plans, and educate patients and families while documenting progress under physician supervision.

The role is full-time with compensation ranging from $39 to $45 per hour, depending on experience.

Qualifications

• Must have graduated from an accredited OT program.
• Must hold a valid OT license and state registration.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Ability to perform evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service and professional conduct.
• Compliance with confidentiality and Residents' Rights.

Responsibilities

• Assess referred patients to determine rehabilitation needs and provide direct OT care.
• Establish and administer a treatment program with specific goals based on patient capacity.
• Document patient condition and educate patients and families in therapy methods.
• Oversee and supervise Occupational Therapy Assistants.

Occupational Therapist

Luxor at Mills Pond Rehabilitation & Nursing Center is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: FULL TIME

Compensation: $39-$45/hour pending experience.
