Job summary

Manchester Center for Rehabilitation and Healing seeks a dedicated Occupational Therapist to join our growing team in providing comprehensive rehab services to residents. The OT will assess patients, develop and administer individualized treatment plans, and set attainable goals aligned with patient capacities and physician directions.

Responsibilities include documenting patient progress, educating patients and families, supervising OT Assistants, and collaborating with interdisciplinary teams

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience supervising Occupational Therapy Assistants is required.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation skills and ability to write evaluations, progress notes, re-certifications, and discharge documents.
• Collaborates with all disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients to determine rehabilitation needs and medical condition.
• Provide direct patient care and implement a treatment program.
• Set realistic goals based on patient capacity and tolerance.
• Document patient condition and progress, including weekly notes and discharge documentation.
• Educate patients and families on therapy methods and expectations.
• Oversee and supervise Occupational Therapy Assistants.

Occupational Therapist -OT Full Time and PRN

Manchester Center for Rehabilitation and Healing is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
