Job summary

Alternate Solutions Health Network seeks an Occupational Therapist to deliver home-based therapy across Indiana territories including New Albany. You will work with an interdisciplinary team, educate families, and ensure safe, compliant care under physician orders.

Frontline OT duties include care planning, patient evaluation, exercise programs, and documentation. A current Indiana OT license, valid driving credentials, and at least one year of acute care OT experience are preferred.

Qualifications

  • OT licensure in the state of employment is required.
  • Minimum one year of OT experience in an acute care setting preferred.
  • Valid driver's license and auto insurance in your name.

Responsibilities

  • Provide skilled OT services as ordered by the attending physician.
  • Educate patients and families, ensure safety, and document care accurately.
  • Develop and adjust individualized OT plan of care; collaborate with the interdisciplinary team.
  • Supervise and communicate with COTA; follow agency and Medicare guidelines.

You'll provide home-based therapy care for patients as directed by the attending physician. HOW YOU'LL WORK

You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

MAJOR AREAS OF RESPONSIBILITY
  • Plan of Care: Provide skilled occupational therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Develop an individualized OT plan of care and make adjustments as needs change.
  • Evaluation: Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
  • Minimize Patient Risk: Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
  • Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Supervision: Oversee and supervise total care of patient provided by COTA.
  • Communication/Collaboration: Communicate with the COTA on supervisory visit results.
  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Operations: Attend in-service training and mandatory agency meetings.

REQUIREMENTS
  • Occupational Therapist with a current license in the state of employment.
  • Minimum one-year experience as a OT in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver Capable of all physical demands
