Job summary

Creative Health Solutions is seeking a PRN Occupational Therapist to join our growing team. The therapist will assess patients, develop and adjust individualized treatment plans, and collaborate with a multidisciplinary staff to support functional goals in skilled nursing settings.

The ideal candidate holds an MS or BS in Occupational Therapy, is licensed or eligible in Indiana, and demonstrates strong communication, organizational, and documentation skills.

Qualifications

• Master of Science in Occupational Therapy or BS in Occupational Therapy with 5 years direct practice experience.
• Current license or eligible for license in the state.
• Excellent oral and written communication and interpersonal skills.
• Adequate computer skills to complete required duties and documentation.
• Demonstrate ability to assess patient needs and develop a comprehensive plan of care.
• Strong organizational skills to manage multiple projects simultaneously.
• Requires creativity and ability to work independently.

Responsibilities

• Review patient information and evaluate impact on assessment and treatment.
• Conduct and document thorough functional assessments including home and equipment needs.
• Develop and implement an individualized treatment plan with achievable goals.
• Revise treatment plans as needed and document accordingly.
• Provide supervision to assistants as allowed by State Practice Act.
• Communicate with nursing staff, patients and families to ensure effective care.
• Instruct patients and families in exercises and procedures for post-discharge.
• Attend rehab team and other department meetings as required.
• Maintain continuing education and comply with regulatory standards.

We are seeking a skilled and passionate Occupational Therapist to join our team at Creative Health Solutions. The ideal candidate will work closely with a team of healthcare professionals to develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.

Responsibilities

As an Occupational Therapist you will assist in:

• Review available patient information and evaluate the impact of this information on the assessment and treatment process.


• Conducts and documents a thorough functional assessment of the patient utilizing standardized procedures. Includes home/vocational and equipment needs.


• Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals.


• Revise treatment plans, as needed, throughout the treatment process.


• Completes documentation in accordance with applicable federal and state laws and applicable standards and administrative and clinical requirements.


• Provides supervision to Assistants in accordance with applicable State Practice Act guidelines.


• Reports to work as scheduled.


• Orient patients to program services and treatment procedures appropriate to their needs. Inform patients of any potential risk during any procedure.


• Communicates with nursing staff, patient and families to ensure effective care delivery.


• Establish and instruct patient and family/other caregiver in exercises and therapeutic procedures to be continued following discharge.


• Assist in the referral process when a patient requires additional services following discharge. Maintain current knowledge of community resources.


• Consistently records outcome scoring data accurately.


• Consistently utilizes appropriate treatment interventions for each patient served that are individualized and ensure appropriate level of challenge, sophistication, and complexity to facilitate maximal patient progress and outcomes.


• Attend and participate in rehabilitation team conferences regarding patient progress, problems or needs.


• Attend and participate in other department/facility meetings, as required.


• Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical accuracy.


• Act in compliance with CHS policies and procedures, State, Federal and Professional Organization regulatory and professional standards and guidelines.


• Provide input to the Rehab Director on clinical performance of assistants as requested and appropriate.


• Perform other duties as assigned.

Qualifications

Minimum Qualifications/Requirements:

• Master of Science degree in Occupational Therapy or Bachelor of Science degree in Occupational Therapy and five years of direct practice experience.


• Current license or eligible for license under statutes in the state of practice.


• Demonstrates competency in applying the principles, methods, materials and equipment used in Occupational Therapy to the patient population served by this facility.


• Excellent oral and written communication and interpersonal skills.


• Adequate computer skills in order to complete required job duties and computerized documentation.


• Demonstrate ability to assess patient needs and develop and implement a comprehensive plan of care.


• Strong organizational skills in order to effectively maintain multiple projects simultaneously.
