Job summary

Ultimate Therapy Services is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team at Autumn Lake at Calvert Manor in Rising Sun, MD. This full-time role focuses on helping residents regain independence and perform daily activities in a skilled nursing and post-acute setting.

You will evaluate, plan, and implement OT interventions, coordinate with a multidisciplinary team, educate residents and caregivers, and document progress accurately.

Qualifications

• Active Maryland OT license required.
• Strong communication and organizational abilities.
• Experience in skilled nursing or rehabilitation preferred but not required.

Responsibilities

• Perform OT evaluations and develop individualized treatment plans.
• Provide ADL-focused interventions and enhance upper extremity function, balance, cognition, and safety.
• Recommend adaptive equipment and educate residents and caregivers.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with PTs, PTAs, COTAs, SLPs, nursing staff and physicians.
• Complete accurate and timely clinical documentation.

Ultimate Therapy is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team. This full-time opportunity is ideal for a therapist who enjoys helping residents regain independence and improve their ability to perform everyday activities in a skilled nursing and post-acute care setting.

What You'll Do

• Complete occupational therapy evaluations and develop individualized treatment plans
• Provide skilled interventions focused on activities of daily living (ADLs), upper extremity function, balance, coordination, cognition, and safety
• Recommend adaptive equipment and educate residents and caregivers on its proper use
• Monitor resident progress and modify treatment plans as needed
• Collaborate with PTs, PTAs, COTAs, SLPs, nursing staff, and physicians to coordinate resident care
• Complete accurate and timely clinical documentation
