Job summary

The Riverside Premier Rehabilitation & Healing Center is seeking a dedicated Occupational Therapist to join our team. You will assess referred patients, design and implement individualized treatment plans, and document progress under physician direction.

You will also educate patients and families, supervise OT Assistants, and ensure compliance with confidentiality and HIPAA. Candidates should have an accredited OT degree and an active state license; experience in long-term or geriatric care is

Qualifications

• Graduate from an accredited OT program with a valid state license.
• Experience supervising Occupational Therapy Assistants and documenting care.
• Geriatric/long-term care experience preferred; strong patient-centered documentation.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential for improvement.
• Develop and administer individualized treatment plans with clear goals.
• Document patient condition and progress; set realistic goals with patients and families.
• Collaborate with physicians and other disciplines to ensure optimal patient care.
• Provide discharge planning, progress notes, and re-certification documentation, adhering to confidentiality and HIPAA.

The Riverside Premier Rehabilitation & Healing Center is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Rate: 37-45 an hour
