Job summary

Kensington-Gardens Rehab and Nursing Center-2 is seeking a licensed Certified Occupational Therapist (OT) to join our skilled nursing facility rehabilitation team. The OT will evaluate residents, develop individualized treatment plans, and provide occupational therapy to promote independence, safety, and improved quality of life.

Requirements include a Master’s or Doctorate in Occupational Therapy and an active Florida OT license; geriatric rehabilitation experience and strong documentation are

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in good standing in Florida.
• Geriatric rehabilitation and Medicare regulations knowledge helpful.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide therapy focused on ADLs, mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.

Full Time

We are seeking a compassionate and licensed Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team. The Occupational Therapist evaluates residents, develops individualized treatment plans, and provides occupational therapy services to promote independence, safety, and improved quality of life.

Key Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate and timely documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.

• Skilled nursing or long-term care experience preferred.
