Job summary

Creative Health Solutions is seeking a dedicated Occupational Therapist for a full-time position in a Skilled Nursing facility. You will develop and supervise therapy plans, collaborating with a multidisciplinary team to optimize patient outcomes and functional independence.

You will document progress, update treatment plans, and continually educate patients and families on therapy techniques while staying current with best practices in rehabilitation.

Qualifications

• Valid State OT License.
• Experience in skilled nursing settings is desirable.

Responsibilities

• Develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.
• Work closely with a team of healthcare professionals to create personalized treatment plans for patients.
• Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
