Allaire Health Services, LLC is seeking an Occupational Therapist to lead rehabilitation in our skilled nursing facility. The OT will assess mobility, develop treatment plans, deliver therapy, and ensure safe patient mobility with remote monitoring when appropriate.

The role emphasizes collaboration with physical therapists and speech-language pathologists, documentation accuracy, and proactive education for residents and families. CPR/BLS and state licensure are required.

Qualifications

• State licensure as an Occupational Therapist is required.
• Experience with EMR and MS Office applications is expected.
• Strong clinical and interpersonal communication skills are required.
• Ability to supervise assistants, technicians, or student interns is preferred.

Responsabilités

• Conduct comprehensive assessments to determine residents' impairments and mobility needs.
• Develop individualized treatment plans with clear goals for functional mobility.
• Administer therapy interventions including exercises, manual techniques, and modalities.
• Educate residents, families, and staff on safe mobility techniques and ongoing progress.
• Maintain thorough documentation in EMR systems (InnovaHealth/Net Health/PCC).
• Collaborate with rehab team (PTs, SLPs) for comprehensive resident care.

Job Description:

The Occupational Therapist plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Responsibilities:

• Assessment and Evaluation: Conduct comprehensive physical assessments todetermineresidents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
• Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents'functionalmobility.
• Treatment: Administer therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.Conducts remote check-in through remote therapeuticmonitoring(RTM) with discharged patients to home or next level of care.
• Documentation:Comfortable and knowledgeable with Point ofService(PoS) care delivery and documentation.Maintain detailed and up-to-date documentation of resident evaluations, treatment plans,dailyand progress notes, HMO/Manage care progress updatesinEMRs(InnovaHealth/Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with the rehab team, including Physical Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.

Qualifications:

• State licensure asanOccupational Therapist.
• EMR and MS office applications.

• Knowledge of Section GG data collection and documentation requirements.
• Strong clinical and interpersonal skills.Excellent written and verbal communication skills.
