Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one, in-home and senior living care in Oak Creek, WI. Therapists will assess, plan, and deliver evidence-based OT services, promoting independence and safe aging in place.

Opportunities include in-home visits, senior living community settings, and telehealth options to enhance flexibility and continuity of care. The role supports autonomous scheduling and collaboration with an interdisciplinary team.

Qualifications

• Licensed OT in the state(s) of practice or eligible to obtain license
• Degree from an accredited Occupational Therapy program
• Current CPR certification
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one occupational therapy to older adults in home or senior living settings
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that promote mobility, independence, and safety
• Document in the EMR per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote aging in place and safety
• Provide telehealth when appropriate to extend access

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX's EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Occupational Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

• Exerting medium physical effort, including lifting 25-50 lbs occasionally, 10-25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls model
• Wearing PPE when required
