FOX Rehabilitation in North Wilmington, DE is seeking licensed Occupational Therapists to deliver one-on-one care in clients' homes or senior living communities. This role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes within our Geriatric House Calls model.

Clinicians may incorporate telehealth and other virtual care options to extend access, support more patients, and maintain high-quality, patient-centered care.

Formación

• Licensed Occupational Therapist in good standing.
• Able to provide care in private residences or senior living communities.
• Familiar with documentation in an EMR system and regulatory standards.

Responsabilidades

• Provide one-on-one occupational therapy to older adults in homes or communities.
• Perform evaluations and develop individualized care plans with evidence-based treatment.
• Deliver therapy sessions to improve wellness, mobility, and independence.
• Document in FOX’s EMR according to regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care to extend access and flexibility.

Responsibilities

• Provide one-on-oneOccupationaltherapyto older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR systemin accordance withregulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care whenappropriate toextend access and flexibility
