Location: Fremont, CA

Schedule: Monday to Friday, Day Shift (6–8 hours per day)

Start Date: ASAP

Productivity Requirement: 90%

Preferred Experience: Familiarity with Rehab Optima is a plus

Key Responsibilities:

Conduct evaluations and develop individualized treatment plans

Deliver high-quality therapy to help patients regain independence

Maintain accurate and timely clinical documentation

Work closely with interdisciplinary teams to ensure comprehensive care

Qualifications:
