Job summary

FOX Rehabilitation is expanding its occupational therapy services in Monroe, NJ. We are seeking licensed OTs who can deliver high-impact, one-on-one care to older adults within homes or senior living communities.

You will evaluate, plan care, and deliver evidence-based therapy, with the option to incorporate telehealth to extend access and flexibility, while maintaining autonomy over your schedule.

Qualifications

• Must hold valid Occupational Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in private residences or Senior Living Communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility and functional independence.
• Document in FOX’s EMR in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Qualifications

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required

Salary

In New Jersey, the standard base pay range for a Full-Time role is $70,000 - $95,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
