Responsibilities

• Assess and treat patients to improve daily living and functional skills.
• Develop individualized OT treatment plans with measurable outcomes and progress tracking.
• Document assessments, progress notes, and discharge summaries in a timely manner.
• Maintain compliance with regulatory standards and formal guidelines for OT practice.
• Collaborate with physicians, therapists, and caregivers to coordinate care.
• Monitor patient outcomes using established metrics and adjust plans accordingly.
• Demonstrate high productivity with on‑site, in‑person service delivery and accountability for results.
• Lead patient education sessions on home exercise programs and device usage.
• Identify opportunities for professional development and pursue relevant certifications.
• Engage in targeted outreach to build referral pipelines in the local community.
• Participate in team meetings to share results, best practices, and learnings.

Experience/Qualifications

• Current Occupational Therapist license for California.
