Jefferson Health is seeking a Staff Level II Occupational Therapist to evaluate, treat, and discharge plan for a full patient caseload across acute care, skilled facilities, acute rehab, and outpatient settings.

The role emphasizes collaboration with other healthcare professionals, student education, orientation of new staff, and documentation of progress and discharge summaries to support patient recovery and independence.

Qualifications

• Bachelor’s, Master’s, or Doctoral Degree from an accredited Occupational Therapy Program.
• At least 2 years of clinical practice.
• Valid state license in Occupational Therapy.
• NBCOT certification is required.
• CPR certification is required.

Responsibilities

• Provide therapy evaluations, treatment planning, interventions and discharge planning for a full patient caseload in multiple care settings.
• Participate in student education for part- or full-time students.
• Participate in orientation and preceptorship of new/rotating staff.
• Plan, organize and deliver treatment programs to help clients recover and rehabilitate.
• Record and document health progress information including daily treatments, progress notes, evaluations and discharge summaries.

Job description

The Staff Level II Occupational Therapist is responsible for the evaluation, treatment and discharge planning of all patients referred for their given therapy services. The OT is an integral healthcare provider accountable to assist the patient to progress to their highest level of function. The Staff Level II therapist will uphold the mission, vision, and values of Jefferson Health. Works in conjunction with other health care professions to coordinate treatment plans and care.

Responsibilities

• Provide therapy evaluations, treatment planning, treatment interventions and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, and outpatient settings.
• Participate in student education for part‑time or full‑time students.
• Participate in orientation and preceptorship of new/rotating staff.
• Complete education, teaching and professional activities commensurate with position.
• Plan, organize, and deliver treatment programs that help clients/patients recover and rehabilitate.
• Utilize tools and techniques to administer safe therapeutic interventions and trainings for patients and families.
• Record and document clients/patients' health progress information for evaluation, including all therapeutic interventions such as daily treatments, progress notes, evaluations, and discharge summaries.
• Perform other duties as assigned.

Qualifications

• Bachelor’s, Master’s, or Doctoral Degree from an accredited Occupational Therapy Program.
• At least 2 years of clinical practice required.
• Valid state license in Occupational Therapy.
• Valid certification from the National Board of Certification in Occupational Therapy (NBCOT) required.
• CPR certification required.
