Job summary

Evolve Therapy Services is hiring compassionate occupational therapists for an in-house Skilled Nursing Facility located in Tupper Lake, NY. This full-time, part-time, or PRN opportunity offers a sign-on bonus and a chance to impact resident care directly.

You will plan, organize, develop, and direct OT services in accordance with federal, state, and local standards, ensuring high-quality care. Active OT licensure and ongoing professional development are required.

Qualifications

• Must have a Degree in Occupational Therapy
• Must hold an Active Occupational Therapy license in practicing state(s)
• Commitment to ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure and certification
