Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to provide one-on-one care in homes and senior living settings across Hudson, Cuyahoga Falls, and Stow, OH. Build strong patient relationships while delivering evidence-based therapy and advancing mobility and independence.

You will conduct evaluations, develop plans of care, document in an EMR, and collaborate with PTs, OTs, SLPs, and Fitness Specialists to promote aging in place. Telehealth options may be used when appropriate.

Qualifications

• Requires OT licensure in the state(s) of practice or eligibility.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency; CPR certification may be required.

Responsibilities

• Provide one-on-one occupational therapy to older adults in home, senior living, or memory care settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and safety.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with a multidisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote aging in place and safety.

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required

In Ohio, the standard base pay range for a Full-Time role is $60,000 - $95,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
