Job summary

Pinnacle Home Care is seeking an Occupational Therapist (OT) for PRN home health visits in the Ft. Pierce area and surrounding communities in Florida. You will assess patients, develop care plans, and provide hands-on therapy in patients’ homes to restore function and independence.

Qualifications

• Must hold a current OT license (state-specific).
• Graduate of an accredited OT program recognized by AMA/AOTA.
• Registered with NBCOT.
• Valid BLS certification.
• Minimum two years OT experience, home health preferred.

Responsibilities

• Assess functional status and home environment for barriers.
• Collaborate to develop a comprehensive care plan.
• Design treatment program with goals for improved function.
• Teach or retrain daily activities; fabricate splints as needed.
• Provide prosthetic/orthotic device training; maintain records.
• Coordinate with physicians, discharge planners; referrals as needed.
• Supervise Certified OT Assistants per policy and regs.

Service Area: Ft. Pierce, FT. Pierce Beachside, Grant and surrounding areas

Key Responsibilities

• Assess the patient’s functional status (muscle function, endurance, visual coordination, communication skills, self-care, work capacity, etc.).


• Evaluate the home environment for hazards or barriers to independent living and identify necessary equipment.


• Collaborate in developing a comprehensive care plan.


• Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.


• Teach or retrain patients in daily activities lost due to illness or injury, following agency policies.


• Fabricate splints and instruct patients on using adaptive equipment to enhance function.


• Provide training in the use of prosthetic and/or orthotic devices.


• Maintain accurate clinical records and report any changes in the patient’s condition to the physician.


• Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.


• Work alongside physicians and discharge planners to provide health education to patients and families/caregivers.


• Identify patient and family/caregiver needs for additional services and make appropriate referrals.


• Confirm daily visit schedules to ensure seamless coordination with other staff.


• Supervise Certified Occupational Therapy Assistants in accordance with agency policies and state regulations.

Requirements:

Qualifications

• Must have a current OT license (state-specific).


• Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association.


• Registered with the National Registration Examination of the American Occupational Therapy Association.


• Valid BLS certification.


• Minimum two years of OT experience (home health experience preferred).


• Excellent communication, organizational, and time management skills.


• Must have a valid driver’s license and access to a reliable and insured vehicle.


• Ability to visit homes that may not be wheelchair accessible.


• Confirm daily visit schedules to ensure seamless coordination with other staff.


• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.


• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
