Job summary

Enhabit Home Health & Hospice is seeking a licensed Occupational Therapist to join our home-based care team. You will assess patients in their homes, design individualized therapy plans, and guide daily living activities to help patients regain independence and quality of life.

Must be licensed as an OT in the state and graduate from an approved program; one year of home health experience is preferred. We offer competitive benefits, continuing education, and opportunities for professional growth

Qualifications

• Must be licensed as an Occupational Therapist in the state of practice.
• Minimum one-year experience in home health preferred.
• Medicare home health experience is preferred.

Responsibilities

• Help patients in the home environment improve daily living activities.
• Develop individualized therapy plans to restore or compensate for functional losses.
• Guide and educate patients and families to support independence and safety at home.
