Job summary

Adventist Health Gladstone is seeking an Occupational Therapist for our Home Health team in Oregon. The role involves in-home care, restoring physical, social and mental function, in collaboration with an interdisciplinary group.

Responsibilities include conducting evaluations, developing individualized treatment plans, delivering therapy, supervising assistants, and ensuring accurate clinical documentation while meeting program objectives and safety standards.

Qualifications

• Master's degree in OT (or OTD) required; only if license obtained after Jan 2007 or foreign equivalent.
• Occupational Therapy licensure in all applicable states of practice required.
• CPR or BLS certification from an approved vendor required.
• Valid driver's license; must be at least 21 years old.

Responsibilities

• Completes evaluations to influence treatment plans and addresses assessment areas needing attention.
• Provides therapy interventions using established protocols and communicates discharge plans.
• Completes comprehensive clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.

JOB DESCRIPTION

Our home health and hospice agencies are located throughout California, Hawaii, Oregon and Washington. They serve the entire community and offer personalized in-home care, and many have been recipients of Home Care Elite awards.

Job Summary:

Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Job Requirements:

Education and Work Experience:

• Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD): Required; only if obtained license on or after January 2007; or foreign equivalent.

Licenses/Certifications:

• Occupational Therapy licensure in all applicable states of practice: Required
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required
• Valid Driver's License (DL) and must be at least 21 years of age or older: Required

Essential Functions:

• Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
