Job summary

Dartmouth Health Home Care is seeking an Occupational Therapist to evaluate and treat patients in the Upper Valley region. You will work within an interdisciplinary team and coordinate care with physicians and caregivers to ensure high-quality home health services.

Role requires OT licensure in VT or NH, a valid driver's license, and BLS within 90 days. Opportunities include independent practice with team support and ongoing CEUs in a community-focused setting.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Preferred: 1 year OT experience in home health.
• Must hold at least one active VT or NH OT license.

Responsibilities

• Screen, evaluate, re-evaluate and treat patients referred by a licensed provider.
• Coordinate treatment activities with the health care team and patient.
• Provide clinical oversight of COTAs and Home Health Aides.
• Communicate patient progress with physicians and other providers.
